Investigation Summary
Employees #1 and #2 were installing a new chain on the west 72 ft outfeed from a debarker. They were working from a 9 ft by 80 in. by 36 in. Fleco bucket that weighed 1,850 to 2,000 lb. It was attached to a 950 Caterpillar articulating wheeled loader by a 3/8 in. chain and hook run through a double eye welded onto the bucket and grapple. The loader had a grapple end that was closed around the bucket. When the bucket was being lowered it slipped and fell to the concrete. Employee #1 received multiple fractures to his right leg and Employee #2 received crushing injuries to the heels of both feet.
