Employee killed when hits head in fall to floor

At 3:45 p.m. on August 13, 1992, Employee #1 was walking into Building D, Room 38, at Encanto Elementary School in Phoenix, AZ. She slipped and fell, hitting the back of her head on the floor. Paramedics rendered aid, and then transferred her to Good Samaritan Hospital in Phoenix. Employee #1 had sustained a concussion and died.
