Investigation Summary
At approximately 4:00 p.m. on January 16, 1992, Employee #1 and two coworkers were in a 6 to 7 ft deep trench when an approximately 18 in. by 12 in. by 4 in. section of trench sloughed off and struck Employee #1 in the back, knocking him to the ground. Employee #1 was lifted from the trench with the backhoe bucket. A medical helicopter was flown in and Employee #1 was transported to Kingman Regional Medical Center, where he was assessed. From there, Employee #1 was flown to Flagstaff Hospital where he remained until he died 3 weeks later on February 5, 1992, at approximately 2:30 a.m.
