Investigation Summary
While fixing a malfunctioning sprinkler head positioned 14 feet and 5 inches above a concrete pad, Employee #1 stepped on a plank. The plank dislodged, causing Employee #1 to fall to the ground and receive multiple fractures. Employee #1 was not using fall protection or a fall restraint and no guardrails were in place on the elevated work surface. As a result of his immobilization from the multiple injuries sustained in the fall, blood clots traveled from his left leg vein to his lungs. Employee #1 died 3 weeks after the fall.
