Investigation Summary
Employee #1 reported to his first job site, in Bothell, at 4:30 a.m. By 5:30 a.m., he had completed his work and was driving the company 1989 Ford cargo van to his second job site, which was in Monroe. At approximately mile post 8.2 on state route 2 (a straight stretch of just over 1 mile long) Employee #1 drove off the roadway, hitting the shoulder of the road and flipping the van. Employee #1 suffered massive head injuries and died.
