Employee sustains internal injuries in fall

At approximately 5:00 p.m. on April 13, 1995, Employee #1, of R & B Insulation, fell 30 ft from a roof where he was working. He was wearing fall arrest equipment, but was not trained in how to use it and the employer had no plan for its use. He was taken to the hospital, where he was treated for internal injuries, indicated by blood in his urine. He was in the hospital for four and a half days and released. He has not been interviewed.
