Investigation Summary

Investigation Nr: 170101885
Event: 12/12/1994
Employee dies of apparent heart attack

Between 11:00 a.m. and 12:00 noon on December 12, 1994, Employee #1 was loading his Gordon truck at the James River Corp. Plant, Redmond, WA. A second Gordon driver arrived at 1:00 p.m. to load his truck after Employee #1. The coworker talked to Employee #1 briefly, and later reported that he was in good spirits and seemed fine. The coworker looked for Employee #1 a short time later because there was no activity around his truck. He found Employee #1 wedged between the seats of his truck. The coworker sounded the truck's air horn and a James River Corp. employee responded and called 911. The emergency rescue unit responded in less than two minutes and started CPR. Employee #1 was then taken to Evergreen Hospital, Kirkland. He had died of an apparent heart attack.
