Investigation Summary
At approximately 5:45 p.m. on August 4, 1996, Employee #1, of Columbia Funeral Services, was driving a 1991 Chevrolet van eastbound when he struck head-on a westbound 1984 Chevrolet pickup. Employee #1 apparently died as a result of immediate acute myocardial dysfunction. According to the Pierce County Sheriff's Department, there were no other injuries in the accident.
