Investigation Summary

At 5:30 a.m. on July 24, 1996, Employee #1 finished writing a shift report and was sitting at his desk when he felt chest pains and then suffered a heart attack. A coworker administered CPR and an ambulance was called. It arrived at 5:44 a.m. and transported Employee #1 to the hospital, where he was pronounced dead at the hospital at 6:00 a.m. He had been under a doctor's care for several weeks, and this most likely was not a job-related fatality.
