Employee crushed and killed when I-beam toppled over

Employee #1 and two coworkers were moving I-beams from the Ogden machine to a welding area. Employee #1 was operating the controls of a 5-ton crane from the floor of the production area. The crew positioned the 38 ft 7 3/8 in. long by 67 3/4 in. tall I-beam upright on the floor. Employee #1 lowered the hooks from the beam and pushed the west hook over the beam to the east. He then raised the hooks to clear the beam, instead of moving them to the east. As Employee #1 raised the hooks, one of them caught the hip of the I-beam, causing it to topple over. The 4,890 lb beam fell across Employee #1's abdomen, causing massive internal and crushing injuries. He was killed.
