Employee killed when struck by head carriage
At approximately 6:30 a.m. on June 6, 1996, Employee #1 was performing clean-up duties. While he was doing so, the operator of the 6 ft pony rig started the head rig carriage. The carriage struck and killed Employee #1. Neither the pony rig nor the carriage were locked out.
