Investigation Summary
At approximately 6:55 p.m. on April 21, 1995, Employee #1 had just climbed a stanchion of a Morgan scaffold because a ladder or equivalent safe method was not provided to reach the work area. Upon reaching the top of the stanchion, he reached left to get to the walking boards and put his left leg up upon them. At this moment, he lost his grip and fell 18 feet to the ground, sustaining a fractured left wrist and elbow, a radial head fracture, and a forehead laceration.
