Investigation Summary
Employee #1 and a coworker were discussing with the foreman what to do with a pile of gravel. They were standing near a pile of dirt, and a road grader was nearby. The decision was made to move the pile of gravel next to the pile of dirt, using a bulldozer to be operated by Employee #1 and followed by the grader. The foreman then walked to his truck and the road grader started to back up. Employee #1 and the coworker were walking around the dirt pile toward the bulldozer when the left front tire of the grader ran over Employee #1. He was killed.
