Employee killed in fall from roof

Employee #1 was pressure-washing the 4-in-12 pitched roof of a three-story house. He was working near the edge of the roof when he slipped and fell approximately 17 ft onto the concrete sidewalk. Employee #1 died as a result of head injuries sustained in the fall. He was aware that use of fall protection was required, but he chose not to use it.
