Employee killed when crushed under falling load

At 8:30 a.m. on February 25, 1998, Employee #1 was removing a shipping pallet from underneath a load on the forks of a forklift. Employee #1 told the forklift operator to raise the 2,800 lb load so it would clear projections on the pallet. The load was raised to a height of 12 ft when it fell to the side and off the forklift, landing on top of Employee #1. He sustained blunt force trauma resulting in lacerations of the heart, lung, liver, and pancreas; he was killed.
