Investigation Summary
At 12:00 p.m. on May 27, 2016, an employee was using a forklift truck to unload a metal ramp from a flatbed truck and the forklift truck turned over, pinning the employee under the counterweight. The employee was hospitalized and died two weeks later on June 11, 2016. He received serious crushing injuries to the pelvic area.
