Employee injured in fall from order picker

On November 5, 1997, Employee #1 was operating an order picker. While rearranging the orders on the lift cart, he pushed against the load already on the cart to fit in other items. Employee #1 fell from the order picker and sustained injuries that required hospitalization. It had been common practice to have the cart locking mechanism only partly into the slot under the forklift for lazy disconnect.
