Employee's fingers amputated when cut by steel saw blade

Employee #1 and a coworker were removing a piece of vinyl that had become stuck in a machine when Employee #1's left hand became caught. It struck the 10 in. steel saw blade, lacerating or partially amputating his thumb, index, and middle fingers. The machine's point of operation was not properly guarded.
