Employee's finger amputated in chain and sprocket drive

Employee #1 was pulled off the line to do routine maintenance on a conveyor. He had taken the covers off the chain and sprockets and was spraying the rollers when his hand slipped into the moving drive chain. His right index finger was amputated to the first knuckle. Factors contributing to this accident were the equipment was not turned off and Employee #1 was not trained in lockout/tagout. There were also no written procedures for equipment being locked out and tagged out during maintenance.
