Investigation Summary
At approximately 7:00 p.m. on June 9, 1995, Employees #1 through #4 were in a trench 23 ft long, 4 ft wide, and 10 ft 2 in. deep, with vertical walls and unshored sides. They were laying lengths of sanitary sewer pipe from a manhole in the street to an apartment building. The employees had already laid two sections of 10 ft long pipe on the trench bottom when the south face or wall caved in, knocking Employees #1 through #3 against the north trench face and completely burying them. Employees #1 and #2 died in the trench. Employee #3 was rescued, and hospitalized with minor injuries. Employee #4, the backhoe operator and company owner, suffered a heart attack attempting to dig his coworkers out of the trench. He had triple bypass surgery, but died.
