Investigation Summary
At approximately 12:35 a.m. on July 26, 1994, Employee #1 was assisting the operator of an ED Moulder Machine, serial #253-777, located in the door plant. The operator was completing the intermediate spline setup. With all heads running, the operator raised the guard over the heads. One pressure wheel was caught on the fence. The operator decided to move the pressure wheel to the proper side to the side position before lowering it to cut the stock. But before the operator could turn off the power to the head, Employee #1 reached down in the cutting area to adjust the wheel, which then amputated the fourth and fifth fingers of his right hand. The operator failed to lock out the moulder machine before removing the guard.
