Investigation Summary
At approximately 5:30 p.m. on January 12, 1994, Employee #1 was laying pipe in a trench that was 10 ft deep, 20 ft long, and approximately 6 ft wide. Employee #1 had finished laying the pipe and applying lubrication to the joint, and was ready to exit the trench. The northwest portion of the bank caved-in, burying Employee #1 to his forehead. He suffered massive chest injuries and amputation of the right foot. Employee #1 was pronounced dead on arrival at the hospital.
