Employee has three fingers amputated in packaging machine

Employee #1 was in the ground beef department, threading plastic packaging wrap into a Three K P Chub packaging machine, model 42-2CDGNOZD3, serial #210. The machine was equipped with a clear plastic door guard and an interlock device, but when the door was opened, the machine would not automatically stop. To thread the machine, the door was opened, the machine was placed in jog mode, and the button was depressed. There was 22 in. between the point of operation and the jog button. As Employee #1 reached up through the cutting area into the point of operation to pull down the plastic film, the knife activated, amputating three fingers on his left hand. The machine did not have adequate point of operation protection.
