Investigation Summary

Investigation Nr: 170127492
Event: 08/29/1994
Employee dies in fall from roof

At approximately 12:45 p.m. on August 29, 1994, Employee #1 was spreading treated corn along the west edge of the steephouse roof. The roof is not equipped with safety railings or other means to prevent falls to the lower level. Lightning rods that are placed at intervals along the roof edge are tied together by wire cables and bolted to the roof. While spreading the corn, Employee #1 apparently lost his balance and either fell across a rod or tried to grab onto it. Consequently, the base of the rod uprooted and Employee #1 fell approximately 103 ft to the adjacent ground level. He was pronounced dead at the scene.
