Employee killed when caught between mast and chassis

At approximately 11:30 a.m. on March 29, 1994, Employee #1 was changing attachments on a loader. He had removed the bucket and started installing the forklift attachment. Employee #1 connected the forks to the hydraulic rams, then proceeded to pin the forks to the mast. As he tried to insert a pin, the elevated forks lowered, catching him between the mast and chassis. Employee #1 received traumatic head injuries and was pronounced dead at the scene.
