Investigation Summary

Investigation Nr: 170568349
Event: 02/22/1994
Employees suffer minor symptoms of carbon monoxide poisoning

Employees #1 through #9 were working in a remote warehouse, loading materials onto two lp gas-powered forklifts. Employees #1, #5, and #7 developed headaches, dizziness, and nausea after working for approximately three hours. The warehouse was cleared of employees and ambulances were called. The fire department test recorded a 175 ppm level of carbon monoxide inside the warehouse. The warehouse was ventilated until the carbon monoxide was at a lower, safer level. When the two lp gas-powered forklifts were replaced by a single electric forklift, carbon monoxide levels were reduced to 0 to 2 ppm.
