Investigation Summary
A POWER LINE CREW FOR AN ELECTRIC UTILITY WAS INSTALLING A NEW UNDERGROUND SERVICE FOR A CUSTOMER. ONE OF THE POWER LINE WORKERS WAS WORKING FROM AN INSULATED AERIAL LIFT BUCKET, CONNECTING A FUSE HOLDER AND LIGHTNING ARRESTER ON A UTILITY POLE CARRYING THE 7200-VOLT, SINGLE-PHASE SUPPLY FOR THE SERVICE. THE POWER LINE WORKER WAS ELECTROCUTED. BECAUSE OF BURN MARKS ON THE FUSEHOLDER AND BURNS ON BOTH HIS HANDS, IT IS BELIEVED THAT THE EMPLOYEE TOOK BOTH SIDES OF THE FUSEHOLDER IN HIS HANDS AFTER IT WAS CONNECTED TO THE POWER LINE. THE OTHER CREW MEMBERS WERE WORKING ON OTHER PORTIONS OF THE SERVICE, SO THERE WERE NO WITNESSES TO THE ACCIDENT.
