Investigation Summary
Employee #1 and a coworker were using a lineman truck to unload 80 ft long poles from a semi-truck and pole trailer. They had unloaded four poles and had placed the base end of the fifth pole on the ground. They were lifting the top end of the fifth pole over the side stake on the front of the pole trailer with the boom on the lineman's truck. The pole, which weighed approximately 2,600 lb, caught on the top of the side stake. Employee #1, who was operating the boom truck controls, used the boom and cable to free the pole from the stake. The pole swung free, striking and crushing him against the boom truck. Employee #1 was killed.
