ELECTRIC SHOCK - DIRECT CONTACT WITH ENERGIZED PARTS

A BUILDING SERVICES SUPERVISOR AND ANOTHER EMPLOYEE WERE ASSIGNED TO CHECK A 120-VOLT RECEPTACLE THAT WAS REPORTED TO BE WITHOUT POWER. THE FUSE BOX FOR THIS RECEPTACLE WAS LOCATED IN A FENCED-IN AREA AND WAS ATTACHED TO A LIGHTING TOWER USED TO SUPPORT LIGHTING FIXTURES FOR A BASEBALL FIELD. THE EMPLOYEES DISCOVERED THAT THERE WAS NO POWER TO THE FUSE BOX. THE SUPERVISOR DECIDED TO CHECK ONE MORE THING. HE STEPPED ONTO A CROSS-MEMBER ON THE LIGHTING TOWER. THE CROSS-MEMBER WAS LOCATED ABOUT 4 FEET ABOVE THE GROUND. THE SUPERVISOR HELD A PENCIL-TYPE VOLTAGE DETECTOR IN HIS LEFT HAND AND USED IT TO CHECK FOR VOLTAGE BY THE TRANSFORMERS MOUNTED ON THE TOWER. THE COWORKER HEARD A VERY LOUD EXPLOSION AND SAW THE SUPERVISOR FALL BACK AGAINST THE FENCE. (OTHER WITNESSES ALSO HEARD THE EXPLOSION AND SAW A BRIGHT FLASH OF LIGHT.) APPARENTLY, THE SUPERVISOR HAD CONTACTED AN EXPOSED ENERGIZED PART WITH HIS CHEST WHILE HIS RIGHT HAND WAS AT A DIFFERENT POTENTIAL. HE RECEIVED AN ELECTRIC SHOCK AND DIED OF HIS INJURIES 14 DAYS AFTER THE ACCIDENT.
