Investigation Summary
Employee #1 was loading pieces onto a workbench that was located next to a Danley 110 mechanical power press, #5021. She was moving the material when she tripped on the side of the press and fell. She put her hands into the point of operation to stop her fall and accidentally stepped on the activating foot pedal, which did not have guarding on its sides. The ram came down, amputating the four fingers on each of her hands. Employee #1 had removed the pull-backs to move the material.
