Investigation Summary
On or about February 7, 1996, Employee #1 was operating a 25-ton L & J full-revolution mechanical power press. The employee was seated and stamping parts when a part became stuck in the point of operation. Employee #1 was reaching in with his right hand when he stood up and apparently stepped on the foot pedal, activating the press. His index and middle fingers were amputated to the first joint, and his ring finger was fractured.
