Investigation Summary
At approximately 9:30 a.m. on September 29, 1994, Employee #1 cut a tree prior to its felling and walked to one side. When the tree fell, large limbs struck the ground and caused the tree to spring sideways and strike Employee #1. He was transported to the hospital with trauma to his abdomen and pelvis. Employee #1 died on October 26, 1994.
