Investigation Summary
Employee #1 inadvertently placed a part in a lathe identical to his own, resulting in an incorrect reading. He turned off the lathe and removed the explosion-proof motor frame. He had sought help but then accidentally hit the START button, exposing two cutting collets that flew out and struck him in the chest. Employee #1 suffered heart failure and died. Employee #2 also sustained injuries in the accident.
