Employee killed when struck by stack of falling waferboard

At 8:15 a.m. on June 4, 1996, Employee #1 was restacking lumber at the west end of Building #3, an outdoor storage shed at Sellersburg Home Center, Inc., a wholesale/retail lumber yard and hardware center. He was holding two 2 by 4s when a 2-ton bundle of 4 ft by 8 ft OSB waferboard fell forward off the forks of a Nissan powered industrial truck and struck him, driving the ends of the 2 by 4s into his upper left chest. He sustained internal bleeding and died of asphyxia. The 16-year-old employee had started his first day of work an hour before the accident.
