Event: 10/28/1992
Employee dies after fall with ladder

On October 28, 1992, Employee #1, of Lewis Steel Co., was to remove splice plates from crane rails and replace them with new splice plates. A 36 ft extension ladder was set up on an I-beam approximately 20 ft above the concrete floor. Employee #1 attached the splice plates to a hand line and a coworker on the I-beam pulled them up and started installing them. The coworker heard a noise and saw Employee #1 and the ladder falling. Employee #1 was taken to Heritage Hospital, where he died.
