Investigation Summary
On August 28, 1993, Employee #1, a site supervisor for Rosentreter Excavating, was laying pipe in a trench. A six inch diameter sewer pipe was being installed into an unsupported trench that was approximately 21 feet deep by 150 feet long by 40 inches wide at the bottom. The bottom 14 feet of the trench were nearly vertical and had 86 degree sides. While he was excavating for the first pipe of the day, the west side of the trench caved in and buried Employee #1, killing him. Two citations were issued for serious violations of R408.40941(1) and R408.40114(1).
