Employee dies after being pinned against forklift

Employee #1 was moving bales of corrugated boxes with a forklift in the warehouse. With the forks raised about four feet and the forklift running, he put the gearshift in neutral, applied the parking brake, and dismounted the forklift. A bale started to tip, and as he tried to get out of the way, he ran into the raised forks. He was hit by the bale and pinched against a fork. The employee walked to the office and was then transported by ambulance to the hospital, where he later died of internal injuries.
