Employee dies after falling in restroom

On November 11, 1990, Employee #1, of General Motors Buick City, entered the restroom and walked through a puddle of water on the floor on his way to the urinal. Skid marks were found on the floor and on the urinal, suggesting that his feet slipped out from under him. The doctor's report states that his death was due to trauma to the back of the head.
