Investigation Summary

Investigation Nr: 170210272
Event: 03/31/1992
Employee killed in fall from roof

At about 7:00 a.m. on March 31, 1992, a three-man crew began work on the roof of the Capitol. The project had started the day before. They were to install sheet metal on the ledges of the west roof wing. Employee #1 and a coworker went up to the gangbox on the south roof and put on their safety belts. When they arrived at the west roof, Employee #1 walked back to the south roof and went down the roof hatch to check on electric power for their tools. The coworker saw Employee #1 walking back so he went over the edge, where he did not have a clear view of the south or west roof, to start work. He then heard the sound of a hard hat hitting something and went to check. He looked over the edge and saw Employee #1's hard hat on top of the scaffold about 20 feet below and then saw Employee #1 on the ground near the southeast part of the alcove. Employee #1 fell 100 feet and was dead on arrival at the hospital. No one witnessed the fall. Three serious citations were issued, one of them willful.
