Investigation Summary
On July 17, 1991, Beech Construction was removing and replacing a 10 inch high pressure gas main. After Consumers Power shut down the main and purged the line, Employee #1, a welder for Beech Construction, was to cut and remove a 6 inch piece of the main. This was done on the north side of the vault where the shut off was located. There was approximately 360 psi on the line at the shut off. When the 6 inch piece was being removed, the force on the shut off valve caused the 10 inch line to disengage from a dressler coupling, and blew the shut off and vault into the employee, killing him.
