Employee dies in fall through skylight

On August 22, 1991, a crew of six was doing new maintenance roofing on a warehouse. Employee #1 was walking backward with a mop bucket and apparently tripped. He was killed when he fell through a 4 ft by 4 ft plastic dome skylight 30 feet to the concrete floor.
