Investigation Summary
At 6:00 p.m. on April 28, 2016, a worker was descending a ladder when his hand missed a rung and he fell from the ladder, striking his head on a brick-paved driveway below. The worker died as a result of the head injury he sustained. The incident investigation revealed that the worker had been carrying materials while ascending the ladder.
