Employee killed during loading of oil field equipment

At approximately 11:25 a.m. on December 19, 1990, Employee #1 and two coworkers were loading a skid-mounted motor onto a flatbed trailer. Approximately 11 feet of the skid was loaded onto the rear of the trailer using the truck-mounted loading winch. The loading operation was stopped at this point to hook up the unloading winch cable to an I-beam underneath the skid. Employee #1 and a coworker properly completed this task and then cleared the area under the skid. For some reason, Employee #1 went back under the skid after the signal to load had been given to the winch operator. His head became caught in the pinch point between the I-beam and a roller on the rear of the flatbed trailer. Employee #1 sustained a fractured skull and was killed.
