Investigation Summary

At approximately 10:00 a.m. on March 8, 1993, Employee #1 was on a roof slope taking pre-cut material from a material handler platform. He lost his balance, slid on his back a few feet, and tried to grab the platform. Employee #1 turned 180 degrees, striking his head on a bolt supporting a roof beam, then slid approximately 16 feet down the roof and dropped 12 1/2 feet from the eaves to the ground. He hit the ground head first and was killed. The roof slope was 45 degrees with a 12:12 pitch. Employee #1, who was 53 years old, was in very good health at the time of the accident and had worked in the construction field for most of his life. He was not using a safety belt at the time of the fall, and did not have a catch platform.
