Investigation Summary

At approximately 7:30 a.m. on November 9, 1990, Employees #1 and #2 were conversing casually in a truck yard area while waiting for shift assignments. They were leaning against the front axle of the rear trailer of a triple-trailer transport; the transport was running and its driver was in the cab at the wheel, waiting his turn to proceed into the tire changing area. When the repair/change area was cleared, the driver checked his rearview mirrors, released his brakes, and moved forward. Another transport driver saw the vehicle start to roll toward Employees #1 and #2 and immediately radioed to the driver to stop. Before the transport could be stopped, the front tires of the trailer rolled over the two employees. Employee #1 was struck again by the rear axle and suffered a fractured skull. He was hospitalized. Employee #2 sustained bruises and contusions to his right foot.
