Investigation Summary
Employee #1 and a coworker were hanging a light steel member approximately 60 ft above ground level. The employee had his safety belt around the horizontal member. When Employee #1 unbolted the member due to difficulty on the other end, it swung away from the building and collapsed in the middle, where it was suspended from the crane. Employee #1's safety belt then slipped off the member and he fell approximately 60 ft and was killed.
