Investigation Summary
Employee #1 had apparently placed his head through the glass window of a freight elevator shaft on the fifth floor of an eleven-story hotel when he was struck and killed by the descending elevator. The elevator had no call buttons to summon it to a particular floor. The most common way to locate it was to look through the glass window of the landing door on each floor. A pane of glass was missing from the door on the fifth floor.
