Investigation Summary
At approximately 8:30 a.m. on April 20, 1992, Employee #1 jumped approximately 4 ft down from a roof to a two point suspension scaffold platform to join Employee #2, who was setting a plumb line for parapet brick work. When Employee #1 landed on the platform, the anchorage for the scaffold on the east side, a 16 ft outrigger beam, counterweighted on top of an 8 ft long by 6 ft 6 in. wide manually propelled mobile scaffold panel, overturned and fell approximately 30 ft to the ground. Employee #1 was wearing his safety belt, but because he was not tied off to his lifeline, he fell to his death. Employee #2 was tied off. His fall was restrained and he sustained no injuries. Employee #2 was rescued by coworkers.
