Investigation Summary
Employee #1, a bookkeeper for the Order of the Eastern Star, was apparently trying to push open the doors of an elevator when she fell 28 ft to the bottom of the shaft and was killed. She had reportedly been helping use a rod to defeat the crossbar on the elevator's doors when they opened and she fell. The elevator indicator showed it to be at the mezzanine level.
