Investigation Summary

Investigation Nr: 200012193
Event: 06/08/2005
Employee Is Killed in Fall While Trimming Tree

An employee was trimming trees near an overhead power line. After trimming some branches from an aerial lift, he climbed a ladder to remove a few small branches. He climbed onto a branch located approximately 6.5 meters above the ground. He was wearing a safety belt and tagline, but not a climbing rope. He slipped from the branch and fell. The knot on the tagline failed to stop the employee's descent, and he fell to the ground. (The knot was no longer in place after the fall.) The employee died of injuries sustained in the fall.
